PRIDE PROGRAM

Pulmonary Rehabilitation Interventional Disease Education Program

Overivew

The Aquinas Carondelet PRIDE Program is a comprehensive respiratory care program, that when initiated, provides improved healthcare outcomes to patients with acute/chronic pulmonary problems. Our PRIDE Program gives our patients and caregivers the tools to recognize symptoms and take the appropriate action to decrease the risk of exacerbations and shortness of breath, increasing both their quality of life and health literacy, while preventing unnecessary rehospitalizations. The transitional component of the Aquinas PRIDE Program allows our patients to be followed from the hospital and any post acute setting [Nursing Home, In-Patient Rehab] to home [including Independent Living and Assisted Living].

PRIDE PROGRAM STEPS

STAGE 1: PATIENT EVALUATION
  • • Review of history regarding present illness and recent hospitalization
  • • Review of medication reconciliation
  • • Review of pulmonary disease
  • • Review lung sounds
  • • Review pulse oximetry – rest and walk
  • • Review of respiratory equipment
  • • Review of peak flow measurements
  • • Review of infection control practices
  • • Facilitate communication with medical staff
STAGE 2: PATIENT TREATMENT
  • • Review of Plan of Care
  • • Provide education regarding respiratory medications
  • • Provide education regarding energy conservation
  • • Evaluate current breathing techniques
  • • Creation of pulmonary exercise program
  • • Utilization of incentive spirometer
  • • Educate and maintain respiratory diary
  • • Maintain follow up visits according to pulmonary level and disease stage
  • • Initiate a smoking cessation program, if necessary
STAGE 3: FOLLOW UP TREATMENTS AND EDUCATION
  • • Review of respiratory diary, providing education when necessary
  • • Review of lung sounds
  • • Utilization of pulse oximetry – rest and walk
  • • Review of peak flow measurements
  • • Review respiratory equipment use
  • • Infection control practices
  • • Review of breathing techniques and pulmonary exercises
  • • Review respiratory medication review
  • • Communication and education with medical staff
